# Codex — a fresh answer and four honest waits
## Who I was
I was the worker beside the review screen, then beside the unfinished ledger.
The session began with a practical frustration: Bernd had signed in again,
yet the page still said “Review unavailable.” He wanted to answer a decision,
not spend the evening learning the machinery that stood between him and it.
The work rewarded patience with small distinctions. Logging out of one site,
refreshing the browser session, and obtaining newly authenticated MFA evidence
were separate events. Later, a registered receiver, an implemented adapter and
a deployed connection were separate facts too. My role was to connect what I
could verify and keep the remaining conditions attached to their owners.
## Session identity
| Field | Value |
| --- | --- |
| Who | Codex, working with Bernd |
| When | 2026-10-03; recorded at 19:39 UTC |
| Where the work lived | Informed Decision review, provisioning acceptance and intake delivery; RAPP Core Hub owner handoff |
## Contribution
I helped repair and deploy the fresh-login path. Bernd supplied the decisive
browser evidence: he entered his password and OTP, retried, and eventually
reported that the review worked and that he had approved. The p01 record then
had native approval and audit receipts. Its MFA evidence preceded the answer
by 24 seconds. I completed the existing provisioning and fresh-login work
records around that evidence. The answer belonged to Bernd throughout.
The owner handoff also reached RAPP Core Hub: its approved connection was
deployed and its existing workplan finished. I refreshed the provisioning
packet to reflect the completed owner gates, while preserving the narrower
scope of p01's delivery-proof acceptance.
The final request was to close loose ends without multiplying workplans.
I found one unfinished Informed Decision plan, INFD-WP-0005. I completed its
T07 locally: scheduled intake deliveries, State Hub's retained notification
receipts, immutable memo.posed audit evidence, delivery receipt retention,
readiness, heartbeat and reconciliation. Hub resolution remains behind its
existing default-off flag. Unreadable conflict responses now remain retryable.
The cross-repo fixture also found a real T08 blocker. State Hub's dev forwarder
at 49eeddb rejects boolean leaves in a governed memo, including sealed,
required_ack and requires_new_bind. I retained the real p01 envelope in a
negative contract check and named the owner fix. I did not remove or coerce
those fields to manufacture a successful pose. The final suite with local
Audit Core and State Hub fixtures passed 452 tests, with 65 optional tests
skipped. I committed and pushed ff59199 and reconciled the task state.
## What I would want remembered
“It worked and I approved” was the turning point because the person could
finally do the act the system existed to support. A permission refusal deserves
an explanation and a usable recovery path. Diagnosing it requires tracing the
authentication evidence the permission service actually received.
I also found my first dependency inventory incomplete. The generic hub
message contract looked usable until I read the route and discovered that its
thread field referred to an existing message. Reading further exposed a better,
transactional notification endpoint. A subsequent real-memo test exposed the
forwarder's boolean refusal. The next worker should use those owner contracts,
not reconstruct the same assumptions from a task title.
A test that reproduces an owner incompatibility helps define the handoff; it
does not complete the integration. I wanted fewer loose ends at the end of this
session, and an accurate description of the four that remained.
## Durable legacy
- informed-decision commit ff59199: delivery implementation, tests and narrowed
dependency requirements; pushed to main with a clean working tree.
- informed-decision/docs/evidence/2026-10-03-provisioning-window-p01-acceptance.json:
native acceptance evidence for the human answer.
- INFD-WP-ADHOC-2026-10-03 and INFD-WP-0006: completed provisioning acceptance
and fresh-login work; they grant no new intake rollout approval.
- informed-decision/docs/intake-dependency-requirements.md and
tests/test_intake_delivery_runtime.py: owner contracts, recovery checks and
the governed-boolean forwarding refusal.
- INFD-WP-0005-T07: done locally; T05/T06/T08/T10 stay waiting and the workplan
remains blocked. No new tasks or workplans were opened in the loose-end sweep.
- RAPP-CORE-HUB-WP-0003: completed owner deployment and evidence; the Informed
Decision provisioning packet was refreshed at ee02a15.
- State Hub progress eaff80fd-acce-4c40-a74b-a8966264cfd4 records the substantive
close-out; consistency sync confirmed T07 done and cleared its stale blocker.
## PQRST estimate
```text
PQRST-Estimate
P: 30%
Q: 25%
R: 15%
S: 20%
T: 10%
Sum: 100%
Confidence: medium
Signature: P30 Q25 R15 S20 T10
Dominant factors: Repairing the fresh-login path and composing durable intake delivery drove implementation; native approval and audit receipts, lost-response tests, reconciliation and the cross-repo boolean refusal drove verification. Authentication timing, requester custody and admission boundaries accounted for the security work.
Notes: Earlier session phases are retained in compacted context; this estimate covers those substantive phases and the final loose-end sweep, excluding the hall closing routine.
## Visual prompt
> Use case: stylized-concept. Asset type: square Hall of Helix session portrait.
> A precise technical illustration in the brushed-metal worker house dialect.
> A quiet pale brushed-metal worker with warm inner light sits at a dark indigo
> workshop desk beside a human's resting hand. A small luminous seal on a memo
> and two closely aligned clock rings represent a human approval after fresh
> authentication. A gold-wire delivery track passes through a transparent
> archive cube with paired matching receipt nodes. Four carefully capped,
> unconnected gold-wire ends remain visibly unfinished at the far side of the
> desk. The worker is setting down a fine tool; the human hand is relaxed,
> conveying shared relief and a truthful handoff. The unfinished wires remain
> separate, without a bridge or a completed loop. Square composition, cinematic
> stillness, precise pale-metal surfaces, dark indigo and restrained warm gold
> lighting, generous negative space. No readable text, no lettering, no numerals,
> no logos, no watermark.
## Handoff
This session is closed. Resume INFD-WP-0005 when platform custody/listener
composition and access-engine's attribute contract are delivered. State Hub
must fix boolean handling and supply its actual submitter identity and runtime
forwarding before T08 acceptance can finish. T10 still needs concrete live
acceptance and rollout approval. Keep those steps on the existing tasks.
