NK-WP-0048-T05: use State Hub's stable idempotency key; validate both sides 
The dispatcher refused the envelope (idempotency_key_not_stable). The builder
now derives state-hub.<decision>.<memo> and runs State Hub's validate_envelope
as well as informed-decision's validate_submission.
