informed-decision is the live approver surface but its intake always creates
the approval with actor informed-decision and refuses a pinned approval_id;
approval-engine lets only the actor create; the executor and BEA require
actor service:nk-bind-executor. Options A (INFD presents an existing
approval, recommended), B (relax actor binding), C (founder CLI approver).
