b9997d6fc7 CUST-WP-0074-T02: task wait qualifiers in the hub read model
Adds tasks.decision_id and workplan_dependencies.from_task_id (migration
e8f9a0b1c2d3). A from_task_id column is needed rather than
from_workplan_id plus description: the partial unique indexes key on
(from_workplan_id, target, relationship_type), so two tasks in one
workplan waiting on the same target could not both be indexed, and the
derived wait_kind needs per-task attribution. The two existing unique
indexes are narrowed to frontmatter edges (from_task_id IS NULL) and two
task-origin counterparts are added; downgrade deletes task-origin rows
before restoring the old indexes.

POST /workplans/{id}/dependencies/ accepts from_task_id (must belong to
the from workplan; a task cannot depend on itself). TaskCreate/Update/Read
carry decision_id.

Derived read-model fields, no new write routes (rule 5):
- TaskRead.wait_kind: external | human | both | unqualified, null unless
status is wait (dependency rows from this task / needs_human).
- WorkplanRead.blocked_kind: human | external | none, null unless status
is blocked. Human wins over external.
Both come from api/services/wait_kind.py, applied on GET /tasks/,
GET /tasks/{id}, GET /workplans/ and GET /workplans/{id}.

The identifier-migration reference-count proof now includes from_task_id
(FK count guard 22 -> 23).

STATE-WP-0092: persist flavor from files, treat depends_on as the C-20
canonical edge, exclude flavor=residual from summary/next_steps/deps
unless include_residuals is set. Live primary still needs the alembic
revision applied.

8b207a991a feat(tasks): give task rows a canonical record identifier
All checks were successful
CI Smoke / host-smoke (push) Successful in 0s
CI Smoke / container-smoke (push) Successful in 1s
Build and Publish Multi-Context Image / build-and-push (push) Successful in 26s
Every work-record type carried a stable identifier except tasks, whose rows held
only id, workplan_id, title, status and priority — nothing connecting a row to
CUST-WP-0067-T01 in the file it came from. Matching was therefore by title, so a
renamed heading looked like one task vanishing and another appearing, and the
forge-derived reset had to refuse to touch tasks at all.

Adds tasks.record_id (nullable: no migration can invent an identity for an
existing row) and a backfill that reads the pairing from the repository files,
where a task declares both its canonical id and its projection UUID. 5516 pairs
across 121 repositories with zero conflicts; 4456 of 6073 cache task rows
identified.

Diff and reset now key on record_id where present, falling back to a
title-prefixed key so an unidentified row stays visibly unidentified.

Unknown stays unknown: a row the files do not claim keeps no identity and the
reset keeps refusing to act on it, and an existing identity is never
overwritten — a mismatch is recorded as a conflict rather than resolved.

0949d4c0d8 feat(classification-spine): implement STATE-WP-0065 repo-anchored model
Replace the ad-hoc coordination-domain spine with the Repo Classification
Standard: 14 market domains, classification columns on managed_repos, and
workplans anchored by repo_id (topic_id optional).

- Add Alembic migration d8e9f0a1b2c3 with data backfill and workstream→workplan rename
- Add api/classification.py validation and register-from-classification tooling
- Expose workplan-first REST/MCP surface with legacy workstream aliases
- Add C-24 consistency rule and legacy domain frontmatter mapping
- Update dashboard repos page with category/capability/stake filters
- Update orientation docs; mark STATE-WP-0065 finished

c792ab0bc0 feat(tasks): add needs_human intervention flag (CUST-WP-0009)
- Migration b4c5d6e7f8a9: adds needs_human (bool) + intervention_note (text) to tasks
- API: needs_human filter on GET /tasks/; 422 if flagged without note
- 3 MCP tools: flag_for_human, clear_human_flag, list_human_interventions
- Dashboard: interventions.md with amber cards and "Mark done" button
- Policy router + workstream DoD policy (workstream-dod.md)
- Workstream lifecycle docs page + workplan CUST-WP-0010
- CLAUDE.md: add step 4 (run fix-consistency after workplan writes)
- consistency_check.py: promote C-11 unlinked tasks from INFO to WARN

0ea2788943 Add state-hub v0.1 — local-first state service for the Custodian
Implements the first live layer of the Custodian cognitive infrastructure:
PostgreSQL schema, FastAPI REST API, FastMCP stdio server, and Observable
Framework telemetry dashboard.

- state-hub/: full stack (docker-compose, FastAPI, Alembic, MCP server, dashboard)
- 5 DB tables: topics, workstreams, tasks, decisions, progress_events
- 11 MCP tools + 5 resources registered in .mcp.json
- Observable dashboard: Overview, Workstreams, Decisions, Progress pages
- CLAUDE.md: session protocol (get_state_summary / add_progress_event ritual)
- ~/.claude/CLAUDE.md: global cross-project reference to the hub
- scripts/pull_image.py: WSL2 TLS-resilient Docker image downloader
