chore(consistency): write back state hub workstream/task IDs

Browse source 
fix-consistency registered TREV-WP-0001 and TREV-WP-0002 in the hub and
wrote the resulting workstream/task IDs into the workplan frontmatter;
also adds the generated WORK-RECORDS.md index.
